How to make a gift of stocks and other securities
We appreciate your interest in making a donation of securities to Choices. We value your cooperation and your kind generosity. You will require the following details in order to provide a gift of stocks via electronic delivery:

Before making the transaction, please get in touch with us to let us know the precise asset(s), the number of shares, and when it will be moved to our account.
Note: You cannot transfer “book-entry” shares of stock electronically via DTC if you hold them in a Direct Registration System Account (DRS) or through a Dividend Reinvestment Program (DRIP) with a transfer agency like Computer share, BNY Mellon, or another agent. Instead, the transfer form should be obtained from your transfer agent’s website. To do this, send your transfer form with your section of the form completed to: Choices, 1907 W Camelback Rd, Phoenix, AZ 85015. In order to obtain the shares, we will finish our portion of the transfer form and send it to the agent. Please contact us if you need any further help.

For charitable purposes, a gift of stock must be valued according to specific guidelines. The value of a philanthropic donation of stock is calculated by averaging the high and low stock prices on the gift date. Shares of mutual funds are valued using the fund’s closing price on the day the donation is made.
